• Please attach a copy of the DD-214 form

• Please attach a copy of the active duty member’s orders OR a copy of the active duty member’s SCRA Certificate.

• Please attach a PROOF OF ABODE IN NORTH CAROLINA. Proof of abode includes, but is not limited to: copy of lease, a letter from landlord, copy of utility bill, or verification of on campus housing.

• Please attach or submit a hard copy of a certificate of eligibility (COE). If you are unable to obtain a COE, please omit. The Residency Determination Officer will work with the SCO to obtain this information from the VA.

• Please Upload VR&E Form 1905
